Position Title: Travel Licensed Practical Nurse (LPN)
Location: Saint Albans, Vermont
Job Type: Contract (Travel Assignment)
Weekly Hours: 36–40 hours
Schedule: 8-hour and 12-hour shifts available (Day, Evening & Night shifts)
Position Overview
"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Travel Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF) in Saint Albans, Vermont. The LPN will play a key role in delivering high-quality patient care, ensuring resident comfort, safety, and adherence to individualized care plans. This position is ideal for professionals who thrive in a fast-paced, team-oriented healthcare environment.
Key Responsibilities
- Provide direct patient care in accordance with established care plans
- Administer medications and treatments as prescribed
- Monitor patient conditions and promptly report changes to the RN or physician
- Assist residents with activities of daily living, including hygiene, mobility, and feeding
- Maintain accurate and timely documentation of patient records
- Follow infection control protocols and safety guidelines
- Collaborate with interdisciplinary teams to optimize patient outcomes
Required Qualifications
- Active LPN license in the state of Vermont or compact state
- Minimum 1–2 years of experience in a Skilled Nursing Facility (SNF) or similar setting
- Strong clinical and patient care skills
- Ability to adapt quickly in a travel assignment environment
Education
- Diploma or Certificate in Practical Nursing from an accredited institution
Certifications & Documentation
- Current LPN License
- AHA BLS Certification
- COVID-19 Vaccination (as required)
- Updated Resume
- Valid Driver’s License
Preferred Experience
- Experience in long-term care, rehabilitation, or SNF settings
- Familiarity with Electronic Medical Records (EMR) systems
