Care Manager MSW Responsibilities
The Clinical Social Worker collaborates with patients, families, and the healthcare team to address and advocate for the social and emotional needs of patients. Key responsibilities include:
- Conducting psychosocial assessments
- Developing treatment plans
- Implementing therapeutic interventions
- Planning for discharge
- Providing crisis intervention
- Referring to appropriate resources
This role involves working closely with patients and their support systems, as well as interdisciplinary teams both within and outside the organization, to enhance patient care through effective resource utilization and monitoring. The Clinical Social Worker will take on a leadership role to help achieve patient goals and desired clinical, financial, and resource outcomes, guided by the NASW Code of Ethics and our organization's Mission and Vision.
Qualifications for Care Manager MSW
- Master's Degree in Social Work from an accredited institution
- Washington Clinical Independent Social Worker Associate License
- At least 1 year of experience in acute medical or mental healthcare in either inpatient or outpatient settings
Care Manager LCSW Responsibilities
Similar to the MSW role, the Clinical Social Worker in this position partners with patients, families, and the healthcare team to address and advocate for patients' social and emotional needs. Responsibilities include:

- Developing treatment plans
- Implementing therapeutic interventions
- Planning for discharge
- Providing crisis intervention
- Referring to appropriate resources
This role also involves collaboration with patients and their support systems, as well as interdisciplinary teams, to improve patient care through effective resource management. The Clinical Social Worker will lead efforts to meet patient goals and desired clinical, financial, and resource outcomes, adhering to the NASW Code of Ethics and our organization's Mission and Vision.

Compensation and Benefits
The wage range for this position is provided, including both minimum and maximum rates. Additional compensation may be available, such as shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonuses.
