Job Title: Director of Case Management
Location: Bradenton, Florida
Schedule: Full-Time | Days | No Weekends
Position Summary
A leading acute care hospital in Bradenton, Florida is seeking an experienced Director of Case Management to oversee and coordinate all case management operations within the facility. This leadership role is responsible for driving departmental performance, ensuring regulatory compliance, and integrating patient care coordination and discharge planning processes across hospital departments and external agencies to ensure continuity of care and optimal resource utilization.
The Director will provide strategic and operational oversight of daily case management functions and will be accountable for achieving established clinical, financial, and quality outcomes. This position requires strong executive presence, exceptional communication skills, and a proven ability to lead interdisciplinary teams while delivering measurable results.
Responsibilities

Lead and evaluate all departmental operations, including case management models, staffing plans (skill mix & FTEs), onboarding, competencies, and technology utilization.

Ensure compliance with organizational policies, regulatory standards, and care coordination best practices.

Oversee core case management functions, including utilization review, discharge planning, and documentation integrity.

Allocate resources effectively to meet productivity and quality benchmarks.

Collaborate with executive leadership (including CFO and division leadership) to achieve operational and financial goals.

Analyze performance data, identify improvement opportunities, and implement sustainable performance improvement initiatives.

Establish and maintain collaborative relationships with key stakeholders including Medical Staff, Nursing Leadership, Ancillary Services, Ethics & Compliance, and Legal teams.

Ensure timely, high-quality discharge planning and care coordination processes to support optimal patient outcomes and length-of-stay management.
Qualifications

Bachelor’s Degree (Required)

Master’s Degree in Nursing, Health Administration, or Business Administration (Preferred)

Active licensure as one of the following (Required):

Registered Nurse (RN)

Licensed Clinical Social Worker (LCSW)
-
Licensed Master Social Worker (LMSW)

Minimum 3+ years of acute care hospital case management experience (Required)

Minimum 2+ years of case management leadership experience (Required)

Strong leadership, analytical, and presentation skills

Demonstrated ability to drive performance outcomes and lead cross-functional teams
