Position Summary:

Completes all paperwork relevant to position, (including but not limited to: forms, prescription refills and prior authorizations responsible for the coordination of referrals, prior authorizations, ordering Durable Medical Equipment & medication refills throughout Oak Orchard.


Job Responsibilities:

  • Works with the team to assure that Oak Orchard patients are given top priority both personally and professionally. The patients and providers daily needs are met through appropriate systems and the highest level of patient services.
  • Primary communicator with patients regarding referrals, reviews details and expectations of referrals with patients.
  • Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
  • Assemble information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialist.
  • Contact review organizations and insurance companies to ensure prior approval requirements are met for referrals. Present necessary medical information such as history, diagnosis and prognosis as required.
  • Ensure that referrals are addressed in a timely manner including receiving consult notes from specialist.
  • Relates effectively to providers and staff.
  • Maintains accurate documentation on patient’s medical record according to guidelines in “Documentation in the Patient Record”.
  • Process medication refill requests and forward to provider for fulfillment.
  • Follow ConferMed guidelines in processing ConferMed appropriate referrals, ensure all supporting documentation is included.
  • Participates in meetings as assigned.
  • Maintains a safe, clean, orderly work area.
  • Any other reasonable requests by management.

Education/Experience:


· Minimum High School Diploma

· 1-2 years working in a medical office preferred

· Experience with electronic health record systems
