Position Summary
Under the supervision of the Medical Director, the Nurse Practitioner provides expanded care to patients by exercising independent judgment related to patient assessment, diagnosis, and treatment. This includes education, counseling, and comprehensive care in a collaborative, primary care setting.
Key Responsibilities
- Conduct patient histories and physical examinations.
- Order and interpret diagnostic tests, including lab work, imaging, and EKGs.
- Diagnose and treat acute and chronic conditions, prescribing medications and therapies as appropriate.
- Provide health maintenance and preventive care, including health education and counseling.
- Collaborate with interdisciplinary team members, including RNs, MAs, case managers, and medical assistants.
- Make referrals to specialists and arrange for hospital admissions or other follow-up care as needed.
- Maintain accurate and timely patient records in accordance with UHC policies.
- Participate in quality improvement and performance initiatives.
- Provide on-call coverage as assigned and perform related duties as required.
Minimum Qualifications
- Current Nurse Practitioner certification by ANCC or AANP.
- Active DC NP license, DEA license, and CDS registration.
- At least two (2) years of clinical experience in ambulatory care.
- Experience working with underserved populations or homeless patients preferred.
- Willingness to participate in a regular on-call rotation.
Knowledge, Skills & Abilities
- Proficient in diagnostic and clinical procedures common to primary care.
- Familiarity with team-based care and electronic health records.
- Strong interpersonal and communication skills.
- Ability to work with diverse populations in a fast-paced environment.
Supervisory Controls
This position reports directly to the Medical Director.
