A-Line Staffing is now hiring an RN – Case Manager (Home Hospice). This is an on-site, contract opportunity with strong growth potential in a home hospice setting.

RN – Case Manager Compensation

• Pay Rate: $56.14/hour
• Mileage Reimbursement: $0.43 per mile

RN – Case Manager Highlights

• On-site / field-based role
• Contract assignment with potential to convert based on performance and business needs
• Full-time: Monday–Friday, 8:00 AM – 4:30 PM
• Some on-call required as needed
• Territory includes: Williamsburg, Yorktown, Gloucester, Hampton Roads, Poquoson, and Charles City

RN – Case Manager Responsibilities

• Perform comprehensive assessments of high-risk hospice members
• Collaborate with primary care providers and interdisciplinary teams to develop individualized care plans
• Ensure implementation of cost-effective, appropriate levels of care
• Coordinate care transitions and discharge planning
• Arrange non-medical supports to ensure treatment compliance (housing, transportation, etc.)
• Coordinate with physicians, social workers, discharge planners, and claims professionals
• Engage specialty resources to support optimal patient outcomes
• Maintain detailed clinical, functional, and fiscal documentation
• Identify opportunities for health promotion and illness prevention
• Prevent adverse patient events and intervene promptly when needed
• Act as a patient advocate while maintaining privacy and confidentiality

RN – Case Manager Requirements

• Associate’s or Bachelor’s Degree in Nursing or related field
• Current, unrestricted Virginia RN license or Compact license
• Hospice experience required
• Case Management certification
• Strong clinical judgment and assessment skills
• Ability to analyze complex medical information and make sound decisions
• Ability to work independently in the field
• Attendance is mandatory for the first 90 days

Preferred Qualifications

• Experience with Homecare Homebase (HCHB)
• Prior home hospice or community-based nursing experience
