Deliver skilled nursing care in patients homes by coordinating and implementing personalized care plans in accordance with physician orders and agency protocols. This role requires effective management and supervision of Home Health Aides and Licensed Practical/Vocational Nurses to ensure high-quality, compliant home health services. The position is based in OFallon, MO, and offers the opportunity to provide compassionate care in a community setting.
Key Responsibilities:
- Conduct thorough patient assessments and develop individualized care plans.
- Supervise and evaluate Home Health Aides and LPN/LVNs regularly, adjusting assignments based on patient needs.
- Collaborate with physicians and healthcare personnel, promptly reporting changes in patient conditions.
- Maintain accurate, timely clinical documentation in compliance with state and federal regulations.
- Ensure adherence to professional standards and agency policies.
- Attend periodic meetings and provide training or counseling as appropriate.
- Manage travel to patient homes reliably with valid transportation and licensing credentials.
Qualifications & Desired Experience:
- Current, active RN license appropriate to assigned state, with eligibility for multi-state practice under the Nurse Licensure Compact (if applicable).
- Minimum 2-3 years of nursing experience, preferably with home health or community-based care.
- Current CPR certification.
- Ability to assess diverse patient needs, including age-specific conditions.
- Competency in basic computer skills for documentation, electronic communication, and training modules.
- Graduation from an accredited school of nursing; BSN preferred where indicated.
