Are you ready to make a meaningful impact in the lives of seniors? As a Primary Care Physician (PCP), you will lead our dedicated team in a collaborative, team-based care environment. Our multidisciplinary care team includes doctors, advanced practice professionals, Pharm D, care coach nurses, MAs, behavioral health specialists, quality-based coders, referral coordinators, and more. Exciting Compensation Package

*** $60,000 Sign-on Bonus ***

Your total compensation package (base pay + bonus) could exceed $300K, depending on experience and location.

Your Responsibilities

  • Evaluate and treat center patients in accordance with established standards of care.
  • Monitor patient care quality using available data and chart reviews.
  • Coordinate patient services, including specialty referrals and home health care.
  • Actively participate in daily huddles as a key source of medical expertise.
  • Foster a cooperative practice environment with professionalism and approachability.
  • Complete medical record documentation promptly, optimizing coding specificity.
  • Adhere to policies and protocols defined by Clinical Leadership.
  • Engage in discussions about quality of care and outcome data with the Regional Medical Director.
  • Explore growth opportunities for new or existing services within the Center.
  • Participate in the local primary care “on-call” program as needed.
  • Ensure compliance with licensing, certification, and accrediting bodies.
  • Focus 100% of your time on direct patient care, including patient-facing and administrative tasks.

Make an Impact with Your SkillsRequired Qualifications

  • Current and unrestricted medical license or willingness to obtain one in the state of practice.
  • Graduate of an accredited MD or DO program.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills with colleagues and patients.
  • Fully engaged in the concept of integrated team-based care.
  • Willingness to learn and adapt to a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email and EMR.
  • This role is patient-facing and requires participation in CenterWell’s Tuberculosis (TB) screening program.

Preferred Qualifications

  • Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • 2-5 years of directly applicable experience preferred.
  • Experience managing a Medicare Advantage panel of patients.
  • Knowledge of Medicare guidelines and coverage.
  • Bilingual skills are a plus.
  • Familiarity with HEDIS quality indicators.
