Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications, perform treatments, and provide wound care as prescribed by physicians.
- Educate patients and their families on disease management, treatment plans, and self-care techniques.
- Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
- Document patient care accurately and maintain confidentiality of patient information.
- Provide emotional support and counseling to patients and their families to promote well-being and coping strategies.
- Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
Qualifications:
- Current Registered Nurse (RN) license in the state of CA.
- 1-2 years of clinical nursing experience, preferably in Home Health or Hospice; or a related setting.
- BLS/CPR certification required; additional certifications such as ACLS or wound care certification preferred.
- Valid CA Driver’s license
Documentary Requirements:
- Compliance with federal I-9 requirements (please bring suitable documentation with you on your first day of work verifying your identity and legal authorization to work in the United States); and
- Mandatory Physical (within the last 6 months prior to the orientation date). You may bring a note from your physician prior to the Orientation Date
- Mandatory TB (within the last year) OR Chest X-ray (within the last 5 years)
- MMR immunization or titer showing immunity prior to your Orientation Date
- Varicella immunization or titer showing immunity prior to your Orientation Date
- Hep B immunization or titer showing immunity prior to your Orientation Date
- TDAP vaccination or declination prior to your Orientation Date
- Yearly Influenza vaccination or declination prior to your Orientation Date
