Licensed Practical Nurse (LPN) – Long-Term Care (LTC)

Location: Platte, South Dakota
Assignment Length: 13 Weeks
Shift: Nights (5:30 PM – 6:00 AM)
Schedule: 12-hour shifts | Every Other Weekend (EOW)

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a Long-Term Care facility in Platte, SD. The ideal candidate will have strong geriatric nursing experience and be comfortable working night shifts in a skilled nursing environment, delivering high-quality, patient-centered care.

Key Responsibilities

  • Provide direct nursing care to long-term care residents in accordance with individualized care plans
  • Safely administer medications and treatments as prescribed
  • Monitor, assess, and document patient conditions and changes
  • Perform wound care, vital signs, and routine nursing procedures
  • Assist residents with activities of daily living (ADLs) including hygiene, mobility, and feeding
  • Report significant changes in condition to the RN and care team
  • Maintain accurate and timely documentation in the electronic medical record (EMR) system
  • Follow all infection control and facility safety protocols
  • Support resident comfort, safety, and quality of life during overnight care
  • Collaborate effectively with CNAs and interdisciplinary team members

Qualifications

  • Active LPN license (South Dakota or Compact license required/preferred)
  • Graduate of an accredited Practical Nursing program
  • Prior experience in long-term care or skilled nursing preferred
  • Current BLS certification preferred
  • Strong clinical assessment and documentation skills
  • Ability to work night shifts and every-other-weekend rotation
  • Reliable, compassionate, and patient-focused approach to care
  • Strong communication and teamwork abilities
