Job Description

JOB SUMMARY

The Transplant Medication Assistance Coordinator coordinates patient enrollment in pharmaceutical assistance programs (PAP) and/or the Transplant Medication Assistance Program (MAP). Determines patient eligibility based on financial requirements and determines which programs are applicable based on individual patient needs. Assists eligible patients to get critical transplant medications through these programs resulting in overall savings to the hospital. Serves as the point of contact with internal staff and external sponsors regarding program details.

ESSENTIAL FUNCTIONS OF THE ROLE

Coordinates PAP and MAP programs for eligible transplant patients who meet the financial requirements per policy.

Coordinates and facilitates the application process with appropriate medical professionals. Provides advice and assistance to patients to ensure accurate completion of all documentation in the required time frame. Ensures that eligible patients receive critical transplant medications in a timely manner.

Assesses patient's continuing care needs and determines the correct program and reenrollment period to help ensure program compliance and minimize patient deductibles.

Works with pharmacy manager and outside vendors to determine the availability of new programs and additional benefits for the patient population.

Prepares, reviews and distributes reports related to the MAP and PAP programs.

Document PAP medications requested and received.

Maintains a list of local pharmaceutical representatives and the medication assistance programs available.

Serves as a liaison for MAP and PAP programs with internal staff and external vendors. Serves as an educational resource for healthcare staff on current issues regarding transplant medications that are part of the medication assistance programs.

QUALIFICATIONS

- EDUCATION - Associate's or 2 years of work experience above the minimum qualification

- EXPERIENCE - 2 Years of Experience
