About the Role
We are seeking a dedicated Family Medicine Physician to join our outpatient primary care team. In this role, you will provide comprehensive care to patients of all ages, support preventive health initiatives, and help advance our mission of delivering accessible, high-quality care to the community.
Key Responsibilities
- Provide full-spectrum outpatient primary care for pediatric, adult, and geriatric patients
- Diagnose and manage acute and chronic conditions (e.g., diabetes, hypertension, asthma)
- Deliver preventive care, screenings, and wellness counseling
- Coordinate care with nursing, behavioral health, and specialty services
- Document patient visits accurately in the EHR
- Participate in quality improvement and population-health initiatives
- Promote culturally competent, patient-centered care
Qualifications
- MD or DO from an accredited institution
- Board-Certified/Board-Eligible in Family Medicine
- Active state medical license (or ability to obtain)
- Experience in outpatient or community health settings preferred
- Strong communication skills and commitment to health equity
