Job Summary

The Magnolia Case Manager will provide trauma-informed, guest-driven case management services to individuals who have moved out of homelessness and are now living in permanent supportive housing with the goal of helping them achieve long-term stable housing. The services provided include connecting them with resources in the community that will aid in their transition into housing. The Magnolia Case Manager will work with other agency teams to identify common barriers faced by individuals we serve and help with establishing strategies and procedures for addressing needs on an agency-wide scale.

Shift
Monday – Friday, 40 hours

Housing First

  • Assist guests in identifying barriers they face and connect them with appropriate resources in the community.
  • Learn about, understand and communicate a strong belief of the Housing First model.
  • Engage your guests in and encourage housing conversations.

Work with a Case Load

  • Assist guests from a diverse population on your set caseload to set up goals.
  • Work with guests to assess their personal obstacles to self-sufficiency and develop problem-solving skill
  • Focus on best housing and strengths-based practices to meet the unique needs of each tenant.
  • Work daily with all departments to promote a healthy team environment.
  • Seeking out, engaging, and networking with community partners to find housing options and additional resources,
  • Attend weekly team meetings and contribute to case conferencing conversations.
  • Keep accurate records, case management notes, and gather statistical data as required.
  • Participate in special projects as needed.

Learn to Manage Crisis

  • Learn to utilize a network of community resources that will aid you in providing direct, trauma-informed case management services to guests. These services include crisis intervention, harm reduction, outreach, ongoing assessments, goal setting, and creative problem-solving.
  • Be trained to jump into high stress situations in an effective and trauma-informed way
  • Assist with de-escalation and crisis intervention with guests.
  • Collaborate with the crisis clinician and others to find ways to support individuals with mental and behavioral health barriers.
  • Work and train closely with your supervisor who will give direction and constructive feedback.
  • Learn how and why to set and maintain good boundaries, using Trauma-Informed Care as a guideline.

  • A willingness to understand that housing first, harm reduction, low barrier access to resources is best practice and essential to our mission at The Road Home.

Education and Experience

  1. Experience working with diverse populations preferred.
  2. Lived, work or volunteer experience preferred or a degree in a related field

Required Skills and Abilities

  1. Strong interpersonal skills and ability to work with diverse populations.
  2. Excellent writing and documentation skills.
  3. Willing to be a proactive member of a team.
  4. Ability to work from a Trauma-Informed Care framework.
  5. Familiarity with the Housing First philosophy.
  6. Ability to work with community partners.
  7. Self-starter and self-directed.
  8. Ability to accept direction from Supervisor.
  9. Excellent organizational skills and strong computer skills.
  10. Demonstrated awareness of and sensitivity to diverse populations. Ability to contribute to the agency’s commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
  11. Must pass a pre-employment background check and drug screen.

Physical and Equipment Requirements

  • Ability to move 25 pounds.*
  • Able to walk and stand for at least an hour at a time.*
  • The ability and license to drive an agency vehicle. Must be at least 21 and hold a current, valid driver's license to be covered under The Road Home insurance for driving agency vehicles.
