RN Homecare & Hospital Hospice Admissions

We’re seeking an energetic Registered Nurse to provide admission assessments in homecare and hospital settings, educate families exploring hospice services, and ensure smooth transitions into care.

Full-time - 36 or 40 hours per week; Weekdays with occasional weekend rotation - various schedule options available for discussion

What You’ll Do

Homecare Admissions

  • Conduct comprehensive hospice admission assessments
  • Evaluate eligibility and collaborate with the hospice physician
  • Educate patients and families about hospice philosophy and services
  • Establish an initial plan of care focused on comfort and symptom management
  • Reconcile medications and coordinate urgent needs (DME, medications)
  • Communicate clearly with the interdisciplinary team and primary providers

Hospital Transitions

  • Coordinate hospice admissions from hospital to home
  • Perform hospital visits and support seamless discharge planning
  • Order DME and assess medication needs prior to discharge
  • Collaborate with Referral Center & Case Management teams
  • Support hospitalized hospice patients and ensure continuity of care

Documentation & Excellence

  • Complete thorough EMR documentation and Hospice Item Set requirements
  • Communicate concise clinical summaries to care teams
  • Maintain infection control standards and clinical supplies
  • Ensure compliance, accuracy, and timely follow-up

What You Bring

  • Current North Carolina RN license
  • Valid driver’s license & auto insurance
  • 2+ years nursing experience (Hospice or Home Health preferred)
  • Strong clinical assessment & critical thinking skills
  • Knowledge of end-of-life care and the psychosocial impact of terminal illness
  • EMR proficiency and excellent documentation skills
  • Commitment to the Hospice philosophy of care
