A hospital in Rockland County is currently seeking a licensed Nurse Practitioner to join their team for an exciting opportunity at their Pride Center. About the Opportunity: 
<ul>
<li>Schedule: Monday to Friday- Open to full or part time </li>
<li>Specialty: Primary Care- Pride Center </li>
</ul>
Responsibilities: The Nurse Practitioner (NP) will: 
<ul>
<li>Provide direct care and consultations to patients </li>
<li>Serve as a consistent Care Coordinator through the continuum of care </li>
<li>Assess the physical, psychological, spiritual, and social needs of the patient </li>
<li>Direct patients to the next appropriate service and track their progress </li>
<li>Assure that all patients receive appropriate evaluation, diagnosis, treatment, medical screening, and medical evaluation whenever indicated </li>
<li>Perform other duties, as needed </li>
</ul>
Qualifications: 
<ul>
<li>NY Nurse Practitioner (NP) license </li>
</ul>
Desired Skills: 
<ul>
<li>Experience treating the LGBTQ community</li>
</ul>
