Locations- Greene County, Schenectady County, Warren County, Fulton County, NY

(Hybrid - Assisted Living + Telehealth)
Full-Time | No Nights | No Weekends

Key Responsibilities

• Assess, diagnose & treat patients
• Manage chronic/acute conditions
• Order labs/tests & prescribe medications
• Create care plans with an interdisciplinary team
• Document in eClinicalWorks

Qualifications

• Master's NP degree
• Active NY NP license + DEA
• 3+ years experience (geriatrics preferred)
