A hospital in Cheraw, SC is seeking a Physical Therapist to join its outpatient rehabilitation team. This position focuses on delivering direct therapy services that support recovery, mobility improvement, and functional independence for patients in an outpatient care setting.

Position Overview

The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, and providing ongoing therapeutic interventions based on clinical goals and physician referrals. The role emphasizes patient-centered care and requires effective collaboration with other clinical staff to support consistent and coordinated rehabilitation outcomes.

This position requires the ability to manage a varied caseload, apply sound clinical judgment, and adjust treatment approaches based on patient progress.

Key Responsibilities

• Perform outpatient physical therapy evaluations and assessments
• Develop and implement individualized treatment plans
• Deliver therapeutic interventions to improve strength, mobility, and function
• Monitor and document patient progress throughout the course of care
• Collaborate with physicians and clinical staff to coordinate treatment
• Educate patients on exercise programs and recovery strategies
• Maintain accurate and timely clinical documentation
• Ensure compliance with clinical protocols and safety standards

Qualifications

• Graduate of an accredited Physical Therapy program required
• Active and unrestricted Physical Therapy license in South Carolina required
• Current CPR certification required
• Previous rehabilitation or outpatient therapy experience preferred
• Strong clinical reasoning and communication skills required
