Director of Coordination (Home Care)

Brooklyn, NY

$150K | Onsite

Mon–Thu 9:00–5:00, Fri 9:00–4:00

Position Overview

We are seeking an experienced Director of Coordination to lead and scale a high-volume home care coordination operation. This role oversees a layered team structure and is critical to ensuring cases are fully staffed, hours are not missed, and patients receive consistent, high-quality care. The Director of Coordination plays a key role in patient retention, operational stability, and continued organizational growth within a large, fast-paced home care environment.

The Ideal Candidate

  • Proven leader in home care staffing coordination

  • Comfortable managing large teams within a multi-layer structure

  • Experienced working with a high patient census (1,000+; 4,000 preferred)

  • Highly organized, decisive, and process-driven

  • Calm under pressure with strong judgment and follow-through

  • Strong communicator who enforces standards while supporting team performance

Key Responsibilities

  • Oversee a coordination department supporting approximately 4,000 patients

  • Directly manage Care Managers, who in turn manage teams of Coordinators

  • Ensure all cases are properly staffed and service hours are not missed

  • Enforce coordination policies, procedures, and performance standards

  • Monitor patient retention and staffing efficiency metrics

  • Partner with Operations leadership to support growth and quality initiatives

  • Utilize HHAeXchange (or similar systems) to manage coordination workflows

  • Identify gaps, implement improvements, and drive accountability across teams

Qualifications & Must-Haves (Non-Negotiable)

  • Direct experience in home care (required)

  • Staffing coordination experience strongly preferred

  • Prior management of large teams and high patient census

  • Strong understanding of coordination operations and case management workflows

  • Experience working onsite in a fast-paced healthcare environment

  • Ability to step into a senior leadership role with minimal ramp-up
