Hospice RN Case Manager – Mon-Fri Weekdays | Manage Visits From Home!

Location: Winston-Salem, NC (Serving Forsyth County & surrounding areas)

Compensation: $36.00 - $38.00 per hour (Based on experience)

Position Summary & Responsibilities

Provide compassionate end-of-life care to hospice patients in their personal residences and long-term care facilities.

• Clinical Care: Conduct patient assessments, manage symptoms, and execute individualized plans of care in coordination with physicians.
• Collaboration: Serve as a core member of an interdisciplinary team to coordinate resources and optimize comfort.
• Supervision: Oversee CNA and Home Health Aide (HHA) services for your assigned patients.
• Efficiency & Compliance: Maintain precise clinical documentation via EMR. Schedule daily visits efficiently to optimize time and mileage.

Qualifications & Skills

• Education & Experience: BSN preferred. Minimum of 1 year of nursing experience preferred; homecare or hospice experience strongly preferred.
• Licensure & Certifications: Current, unrestricted North Carolina RN license and active CPR certification.
• Requirements: Valid driver's license and automobile liability insurance. Annual flu vaccination required (medical/religious exemptions considered).
• Skills: Proficiency with EMR systems, patient-controlled analgesia (PCA) pumps, and strong clinical assessment abilities.
