Position Overview:

As a Home Health Registered Nurse (RN), you will work collaboratively with your team to develop treatment plans for patients. You will also have the opportunity to creatively utilize resources available in their homes, educate patients and their families, and adapt care strategies to meet the changing needs of your patients.
Service Areas:
Clinton, Washington, Perry, Marion, Jefferson, Franklin, Wayne, Hamilton, and Edwards Counties in Illinois.
Compensation Range: $32.77 - $52.43

Job Summary:
This role involves coordinating, facilitating, and executing an interdisciplinary care plan through the assessment and evaluation of patient conditions and the effectiveness of care, all under the guidance of the attending physician. You will travel to patients' homes, Community-Based Residential Facilities (CBRF), assisted living facilities, hospitals, and other locations to assess patient needs and ensure compliance with home care and hospice regulations.
Key Responsibilities:
- Evaluate and coordinate discharge planning needs with the healthcare team.
- Conduct point-of-care testing as per established policies.
- Engage with physicians regarding care plans and patient status updates, documenting all communications in compliance with regulations.
- Collaborate with multidisciplinary team members and external agencies to ensure effective patient care and education throughout the discharge process or end-of-life care.
- Complete admission documentation and verify insurance details, ensuring timely patient assessments.
- Deliver skilled nursing care as outlined in the care plan, including vital signs monitoring, wound care, IV therapy, ostomy care, blood draws, injections, and comprehensive patient assessments.
- Document visit notes, treatments, and care accurately and promptly, adhering to physician orders and regulatory requirements.
- Work closely with patients, their families, and the healthcare team to create a tailored care plan that addresses individual patient needs, responding to inquiries and concerns as they arise.
- Maintain a state of alertness and safety at all times.
- Perform additional duties as assigned.
Qualifications:
- Graduation from an accredited nursing program or equivalent educational background for licensing.
Experience:
- No prior experience is necessary.
Additional Requirements:
- Must provide proof of motor vehicle liability insurance that meets state minimum requirements upon hire and upon request.
Physical Demands:
- Regularly use speech for effective communication.
- Consistently stand and walk.
- Frequently lift, carry, push, or pull objects weighing up to 25 lbs.
- Engage in frequent sitting, standing, walking, reaching, and repetitive movements of the legs and arms.
- Utilize vision for near and far distances and to distinguish colors.
- Employ smell to identify odors.
- Use hearing to receive verbal communication and detect alarms or machinery issues.
- Drive frequently.
- Perform data entry and keyboard tasks regularly.
- Occasionally lift or move objects weighing between 25-50 lbs.
- Occasionally assist in lifting or moving patients.
- Occasionally bend, stoop, kneel, squat, twist, grip, and perform repetitive movements.
- Rarely crawl or run.

- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Valid Driver's License and Motor Vehicle Report (MVR)
For Missouri:
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Nurse (RN) - Compact State or Missouri Division of Professional Registration
- Valid Driver's License and Motor Vehicle Report (MVR)
For Oklahoma:
