- Provide emotional support and counseling to residents and their families.
- Conduct assessments to identify residents' needs and develop appropriate care plans.
- Facilitate crisis intervention strategies to assist residents in distress.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
- Educate residents and families about available resources and services.
- Advocate for residents' rights and needs within the facility.
- Maintain accurate documentation of services provided and progress notes.
Qualified applicants will have at least 2 years' experience in a Long-Term Care setting, CSW is preferred.
