**Care Specialist (Caregiver) – Job Description**

**Position Title: **
Care Specialist (Caregiver)
**Primary Function: **
Provide clients with companionship, personal assistance, meal preparation, light housekeeping, and transportation as outlined in the Individualized Service Plan. Care Specialists may also perform skilled tasks under RN Delegation, if delegated and trained. All assignments must be completed within the shift unless extenuating circumstances arise.
Key Responsibilities
**Companionship & Engagement: **
- Foster a warm, friendly atmosphere and build genuine relationships with clients.
- Support clients’ hobbies, crafts, reading, writing, and social activities.
- Encourage participation in outings, exercise, and fun, enriching experiences.
**Meal Preparation: **
- Assist with grocery shopping, meal planning, and meal preparation as specified in the care plan.
**Medication Reminders & Support: **
- Remind clients to take pre-dosed medications and observe as they take them.
- *If RN Delegated: * Administer medications (oral, topical, subcutaneous, etc.) per RN Delegation guidelines and client-specific delegation.
**Personal Assistance Services: **
- Support grooming, hygiene, bathing, dressing, movement, exercise, feeding, toileting, and routine hair/skin care as outlined in the service plan.
*If RN Delegated: * Provide specialized care such as feeding tube care, catheter care, blood glucose monitoring, insulin administration (dial-pen), and non-sterile dressing changes, as directed by the RN.
**Light Housekeeping: **
- Maintain a clean, safe environment (kitchen, bathroom, laundry, living spaces) as described in the care plan.
- *Note: * Do not perform tasks outside your scope or the care plan (e.g., heavy cleaning, moving furniture, window washing).
**Errands & Money Handling: **
- Complete errands as requested by the client, provide exact receipts and change, and document all transactions.
**Transportation: **
- Provide safe client transportation if required, maintaining valid driver’s license, insurance, and vehicle.
- Accurately report mileage for reimbursement.
**Documentation & Reporting: **
- Clock in/out and document all activities, care provided, and client updates using the Aaniie Care App/EVV or Telephony system.
- Maintain accurate records, including RN Delegation documentation when applicable.
- Communicate promptly with the office regarding client changes, concerns, or incidents.
**Dress Code: **
- Wear Kind Hearts Home Care uniform and name tag at all times.
- Present a neat, professional appearance.
**RN Delegation (If Applicable): **
- Care Specialists may be delegated by a Registered Nurse (RN) to perform certain skilled tasks on a client-specific basis, including:
- Medication administration (oral, topical, subcutaneous, etc.)
- Feeding tube care and medication administration
- Extensive catheter care
- Blood glucose monitoring
- Insulin administration (dial-pen)
- Non-sterile dressing changes
**Important: **
- RN Delegation is client-specific. You must not perform delegated tasks for any client unless you have received explicit RN Delegation for that individual.
- 100% understanding of your scope and the consequences of exceeding it is required.
- If a family requests a task outside your scope or tries to persuade you to do something you are not delegated for, contact the office immediately.
- RN Delegated tasks require your highest skill and attention. The RN may determine if a Care Specialist is not suitable for a delegated task.
**Training & Development: **
- All Care Specialists receive comprehensive onboarding and ongoing education through **Care Academy** and in-house RN-led training.
- Orientation includes:
- Care Academy courses on essential caregiving skills, safety, infection control, and specialized care topics.
- RN Delegation training for those assigned delegated tasks, focused on understanding client-specific delegation, scope of practice, and compliance.
- Ongoing annual and as-needed training is mandatory for continued employment and skill development.
**Evaluation & Professionalism: **
- Performance, adherence to company policies, and professionalism are reviewed annually.
- Commitment to Kind Hearts’ mission of compassionate, dignified care is expected at all times.
**Other Requirements: **
- Follow all company policies and procedures.
- Refer to the Individualized Service Plan and RN Delegation documentation for guidance.
- Immediately report any client changes, concerns, or discomfort with tasks to the office.
- Maintain confidentiality and respect for all clients and their families.

We love training our team and helping you grow your skills. Flexibility and a willingness to learn mean we can offer you more hours—sometimes up to 40 per week! The more open you are with your availability, the more opportunities you’ll have for full-time hours. After three months of consistent full-time work, you’ll be converted to full-time status (including PTO).
If you need a set schedule, we’ll do our best to accommodate you as we grow and add new clients. We’re looking for people who show up with a positive, responsible attitude—never leaving clients alone or unattended, and always making each day special. That means keeping clients and their homes comfortable, joining in fun activities, helping them get ready, and enjoying time together—whether it’s curling hair, putting on makeup, or going out for a stroll.
