Job Description:
We are seeking an experienced Home Health Registered Nurse (RN) Case Manager to provide skilled nursing care to patients in a home health setting. This is a contract role offering an all-inclusive rate. The successful candidate will be responsible for coordinating care, ensuring quality outcomes, and documenting patient progress using OASIS.
Key Responsibilities:
- Deliver high-quality nursing care to patients in their homes.
- Complete and manage OASIS documentation accurately and timely.
- Collaborate with interdisciplinary teams to develop and manage individualized care plans.
- Conduct assessments, develop treatment plans, and provide direct patient care.
- Monitor patient progress and modify care plans as needed.
- Ensure compliance with applicable regulations and policies.
- Drive between patient homes within the assigned territory.
Qualifications:
- Active and unrestricted RN license (State of Vermont or compact license).
- Minimum of 1–2 years of recent home health and case management experience.
- OASIS experience is required.
- BLS (Basic Life Support) certification required.
- Strong preference for candidates with prior travel nursing experience.
- Must have reliable transportation and be eligible for mileage reimbursement (commute excluded).
- No time-off requests will be accepted during the contract.
