Now Hiring: Bilingual Case Manager (English & Spanish Speaking)
Location: Must be in Doral or West Miami FL
Hourly: $34.13 per hour + mileage reimbursement
Shift: Monday–Friday | 8:00 AM – 5:00 PM
We are currently hiring a Bilingual Case Manager to support Medicaid members receiving long-term care services. This position is ideal for someone who enjoys helping others, working independently, and building strong relationships with patients and families.
This role requires local travel throughout the Doral & West Miami area. Candidates must live in Doral or West Miami area. Must be comfortable traveling to members’ homes, assisted living facilities, and nursing homes throughout the week.
Job Responsibilities:
- Meet with members in their homes or care facilities
- Help coordinate medical care and support services
- Create and monitor care plans based on members’ needs
- Educate members and families about healthcare resources
- Work closely with doctors, nurses, and healthcare teams
- Document visits, care updates, and member progress
- Encourage members to stay engaged in their healthcare and wellness goals
- Identify concerns related to quality of care and report as needed
Requirements:
- Bachelor’s degree in social work or related field required
- Previous case management experience required
- Experience working with geriatric or elderly populations preferred
- Must be fluent in both English and Spanish
- Must be comfortable with travel throughout Miami Dade County
