Position Responsibilities

In the role of Occupational Therapist (OT), you will assess residents’ functional abilities and create customized treatment plans aimed at enhancing their independence in daily activities. You will provide direct therapy, collaborate with interdisciplinary teams, and ensure the delivery of high-quality rehabilitation services that align with our residents’ objectives.


Primary Duties

  • Assess residents’ functional, cognitive, and psychosocial requirements using standardized evaluation tools.
  • Create and execute individualized treatment plans to enhance daily living skills, mobility, and safety.
  • Deliver therapeutic interventions, including training on adaptive equipment, therapeutic exercises, and functional retraining.
  • Inform residents, families, and staff about strategies to support rehabilitation objectives.
  • Accurately document evaluations, progress notes, and discharge summaries in a timely manner, adhering to regulatory and organizational standards.
  • Collaborate with physical therapists, speech-language pathologists, nurses, and other team members to provide coordinated care.
  • Ensure compliance with all relevant state and federal regulations.


Qualifications

  • Active licensure as an Occupational Therapist (OT) in the state of practice.
  • Graduation from an accredited Occupational Therapy program.
  • Preferred prior experience in long-term care, skilled nursing, or rehabilitation settings.
  • Excellent communication, documentation, and organizational skills.
