Nurse Practitioner (NP) / Physician Assistant (PA)

$140,000 - $160,000

Join a growing primary care practice based in Williamsburg providing telehealth visits for patients in assisted living facilities. This role offers a stable, supportive environment and the opportunity to build ongoing relationships with a recurring patient panel. (Not a remote position.)

Responsibilities:

• Provide comprehensive primary care services to geriatric patients via telehealth.
• Conduct virtual visits with medical assistant support at each assisted living facility.
• Review and interpret lab results and diagnostic data collected onsite.
• Maintain accurate and timely documentation using established templates.
• Collaborate with other PAs, NPs, and medical staff to ensure continuity of care.
• Deliver high-quality, compassionate care to patients within an organized telehealth system.

Qualifications:

• Licensed PA or NP in New York State.
• Credentialed with New York State Medicaid and Medicare (preferred).
• Experience in primary care, telehealth, or geriatrics preferred but not required.
• Strong clinical judgment and ability to work independently.
