Job DescriptionDescription:

Reporting to the Director of Social Services and Case Management, this position is responsible for helping those with acute and chronic illness, mental health issues or life-altering conditions to gain access to healthcare services across the continuum. Their duties include meeting with clients and their families, connecting clients with professional services and maintaining consistent communication with clients to obtain data and communicating with the healthcare team. Additional duties include ongoing review of quality data, chart reviews, utilization reviews, authorizations for services, admission coordination, and clinical documentation improvement.

Requirements:

· Graduate of an accredited program in nursing.

· Active State of Ohio Licensed Practical Nurse Nursing License.

· At least 5 years of nursing experience in an acute health care setting

· Good interpersonal, written and verbal communication skills

· Computer literacy


ESSENTIAL FUNCTIONS

· Collect and document objective and subjective data related to the patient’s health and social status and report data to the registered nurse, social worker or health care provider.

· Contribute to the development and maintenance of discharge plans for patients under the supervision of a registered nurse, completing reviews for admission criteria, and ongoing continued stays, as well as intake for potential swing bed admissions.

· Develops and maintains thorough knowledge of referral resources throughout the service area, including specialized medical equipment, pharmaceuticals, appropriate post-hospital/post swing bed services, ambulatory health care referrals or follow-up, Rehabilitation services, Hospice services, Extended Care Services, Non-health care services, Home Health services, and Community based care providers.

· Ongoing quality review of documentation with reporting and data dissemination to the healthcare team.

· Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the interpretation of the quality data as well as best practice and performance improvement efforts.

· Maintains up-to-date knowledge of Medicare, Medicaid, and other third-party reimbursement sources and the potential impact on the patient’s post-discharge needs.

· Maintains accurate and timely records, including clinical and progress notes, according to department policy.

· Participates in planning and implementing quality improvement activities.

· Maintains up-to-date knowledge of field by participating in continuing education activities.

· Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies, and others.

· Adheres to all department/hospital policies.

· Supervises students/ as approved by the director and administrator.

· Provides coverage for case manager’s absence.

· Other duties as assigned.


FUNCTIONAL DEMANDS

Working Conditions and Physical Environment

· Works in a patient care setting where there is potential exposure to blood, body fluids, infectious materials; and electrical, fire, chemical, physical contact, and equipment hazards.

· Fast-paced setting with rapid patient turnover.

· High patient traffic.

· Close-quartered and shared workspace.

· Sufficient mobility to negotiate physical plant.

· Strength and endurance sufficient to perform physically for long periods of time without sitting (four to eight hours). May include walking; lifting 25 – 50lbs with or without assistance; bending; twisting; standing; sitting; reaching; stair-climbing and descending.

· Physical dexterity and psychomotor skills to respond adequately and appropriately to routine and emergency patient care needs in an ambulatory setting.

· Visual and auditory capabilities sufficient to read information and respond to patient care needs.

· Ability to work in a computerized environment that includes prolonged sitting and use of keyboard, mouse, and monitor.

· Cognitive ability to accurately observe, analyze, and interpret general and technical data both verbal and written to accurately communicate this information.
