Company: Twin Cities Physicians.
Compensation: $105,000 to $120,000 annually + autonomy + performance-driven growth.
Benefits: Full Benefits Package (Full-Time Only).
Location: Greater Twin Cities Metro Area, MN.
Schedule: Full-Time | Mobile Rounding Across Assigned Territory.

Position Summary:
We are looking for a Wound Care focused Nurse Practitioner who wants to step into a role with clinical ownership, flexibility, and measurable impact. In this position, you will manage advanced wound cases while delivering primary care through a mobile rounding model. You will build strong relationships within your communities and play a direct role in improving healing outcomes, reducing hospital readmissions, and elevating the standard of care.
Responsibilities:
• Provide on-site rounding, primary care, and advanced wound care services.
• Manage pressure injuries, surgical wounds, diabetic ulcers, and skin integrity issues.
• Drive post-acute recovery, chronic disease management, and preventative care.
• Collaborate with physicians, nursing staff, and interdisciplinary teams.
• Reduce hospital readmissions through proactive clinical management.
• Communicate with families, caregivers, and facility leadership.
• Complete documentation, medication management, and lab coordination.
Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
• Experience in wound care, post-acute care, geriatrics, or primary care preferred.
• Valid driver’s license and ability to travel between communities.
• Strong ability to work independently and manage a field-based schedule.
• Passion for patient-centered, outcomes-driven senior care.
