POSITION SUMMARY
We are in search of an advanced practice provider, specifically a nurse practitioner (NP) or physician assistant (PA) with a background in primary care or internal medicine, ideally with a focus on geriatrics. This role offers the chance to enhance outcomes for residents in long-term care by working alongside a team of healthcare providers and case managers. If you are an advanced practice registered nurse (APRN or NP) or physician assistant (PA) who is passionate about working with the elderly, we invite you to consider joining our team. Our providers primarily operate within nursing homes or assisted living facilities, caring for residents enrolled in specialized Medicare Advantage plans. Your role will involve delivering personalized, coordinated healthcare that enhances quality of life and minimizes unnecessary hospital admissions.
As one of our advanced practice providers, you will utilize your independent judgment to manage patients with both acute and chronic conditions, consulting with a supervising physician when necessary. You will collaborate closely with our facility partners and health plan leadership to implement our national care model while adhering to all relevant federal and state regulations.
Key Responsibilities
Our NPs and PAs are integral to our success and play a vital role in improving the quality of life for residents. Your responsibilities will include:
- Evaluating patients’ medical and mental health needs and developing a proactive care plan that addresses their conditions and any barriers to care.
- Ensuring adherence to local, state, and federal regulations concerning the clinical services you provide.
- Prescribing medications and ordering laboratory tests, diagnostic procedures, and consultations.
- Monitoring patients’ adherence to treatment plans and adjusting those plans as necessary.
- Collaborating with RN case managers as part of a cohesive care team.
Qualifications and Experience
- Required: FNP, AGNP, or PA license.
- Degree from an accredited APRN or PA program.
- Minimum of 3 years of experience in clinical nursing or rehabilitation with geriatric populations.
- Experience with Electronic Health Records.
- Proficiency in Microsoft applications, including Word, Outlook, and Excel.
Supervisory Responsibilities
- May be required to provide training and guidance to facility staff.

Preferred Experience
- Experience in Primary Care with a focus on Geriatric Medicine.
- 3 years in Long-Term Care settings.
Required Education
- Master's degree or higher.
Licenses & Certifications
- APRN Advanced Practice RN license required.
