Edenbrook Hampton Rehabilitation and Nursing Center is seeking a dedicated PRN Physical Therapist to join our team. The PRN Physical Therapist will develop and implement individualized therapy programs to enhance patient performance and independence. The ideal candidate will focus on promoting a healthy lifestyle and physical rehabilitation through evidence-based practices. This role involves working with patients of varying needs and collaborating with other healthcare professionals to provide comprehensive care.

Position: PRN Physical Therapist

Primary Job Responsibilities:

  • Develop and implement individualized therapy programs to restore, reinforce, and enhance patient performance.
  • Identify the need for and make referrals to other healthcare disciplines as necessary to address the comprehensive needs of the patient.
  • Instruct, educate, and train patients and caregivers in skills and functions essential for promoting patient independence, improving quality of life, and diminishing or correcting pathology.
  • Supervise Physical Therapist Assistants, provisional licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
  • Develop personalized care plans using a variety of treatment techniques.
  • Create fitness and wellness programs tailored to the specific needs of patients.
  • Provide quality, personalized, and evidence-based care using proven interventions.
  • Motivate patients during treatment to help them function optimally and reach their rehabilitation goals.
  • Promote clients’ healthy lifestyles by improving their strength, flexibility, balance, and coordination.
  • Continuously evaluate patient progress, monitor outcomes, and communicate updates effectively with the interdisciplinary team.

Qualifications:

  • Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program.
  • Active state Physical Therapy license in good standing.
  • Previous experience in a skilled nursing facility or similar setting is preferred.
  • Strong knowledge of rehabilitation protocols and techniques.
  • Excellent interpersonal, communication, and leadership skills.
  • Ability to work independently and as part of a team.
  • Commitment to providing high-quality, patient-centered care.
