RoleDescription
Inresponse to the May 16, 2025, tornado, Aging Advantage and other servicepartners created the Senior Support Team to provide compassionate,client-centered support to impacted older adults. The Senior Support Team’smission is to help adults 60 and older stabilize and recover from the disasterto the extent possible given available resources. This is a full-time on-siterole for a Disaster Case Manager, Older Adult Specialist located in St. Louis,MO. The DCM serves as a specialized advocate and primary contact fordisaster-affected older adult survivors. Daily tasks include conductingassessments, designing and implementing personalized recovery plans,coordinating with community partners, maintaining case records, and guidingclients through a structured recovery process.
Qualifications
Bachelor’sdegree in social work, public health, counseling, education, or related fieldrequired- Master of Social Work (MSW) preferred
Skillsin assessing needs, developing recovery plans, and coordinating services
Minimumof one year of relevant experience in case management, social services, ordisaster recovery
Experienceworking with older adults and knowledge of their specific needs
Demonstratedability to work effectively with diverse populations in high-stressenvironments
Able towork in a fast-paced, dynamic, collaborative environment
Must beable to work independently and work with minimal supervision in the field
Provideethical and sound judgment when advocating and servicing clients in need
Abilityto work collaboratively with other agencies and service providers.
PrimaryDuties & Responsibilities:
ClientRecovery & Case Management
Serve asthe main point of contact for a caseload of disaster survivors who are olderadults, offering crisis advocacy, resource navigation, and detailed referralservices.
Conductintake assessments to identify unmet disaster-related needs and developcomprehensive, individualized long-term recovery plans.
Maintainconsistent and responsive communication through in-person, phone, or virtualmeetings based on client need.
Facilitateaccess to housing restoration, FEMA assistance, mental health support, andother recovery services.
Dataentry & Documentation
Recordclient interactions, case notes, financial expenditures, progress updates, andreferrals using agency systems.
Adhereto confidentiality guidelines and Safe from Harm protocols.
Coordination& Advocacy
Collaboratewith local recovery groups, community organizations, and other agencies toadvocate for clients and ensure equitable access to resources.
Monitorclient progress, adjust recovery plans as needed, and coordinate escalation foradditional support or funding.
Maintainexpertise in funding available in the community, ensuring clients are connectedto the most effective and timely financial recovery resources.
Travel& Field Work
Visitclients in various settings including homes, agencies, or community locations;reliable transportation is required, with mileage reimbursement provided.
AdditionalRequirements:
TechnicalCompetencies
Proficientin data entry, case management documentation, and knowledgeable in clientmanagement software plus general office tools.
Proficiencyin Google Workspace suite preferred (Docs, Sheets, etc.)
PersonalAttributes
Excellentcommunication, active listening, empathy, adaptability, attention to detail,and crisis resolution skills.
Acommitted advocate, collaborator, and ethical steward of client trust andconfidentiality.
Logistics
Validdriver’s license, automobile insurance, reliable transportation, and capacityto travel for home visits and outreach activities are required.

Type ofposition: Full-time, Exempt
Hours:40 hrs/week
Site:Hybrid – on site + work from home
Duration:Contract limited to 12 months, extension possible if awarded. Opportunity for
continuedemployment after contract ends, as preferred.
