Job Title: RN Care Manager – Emergency Department (ED)
Location: Bronx, NY 10461
Duration: 13 Weeks
Schedule: 10:00 AM – 6:00 PM | 40 hrs/week
Pay Rate: $58–$61/hr
Job Summary:
"Navitas Healthcare, LLC" is seeking RN Care Manager – Emergency Department for an exciting job in Bronx, NY.
Key Responsibilities:

Coordinate ED patient care from admission through discharge
-
Perform utilization review and discharge planning
-
Collaborate with physicians, nurses, social workers, and ancillary staff
-
Arrange post-acute services, community resources, and follow-up care
-
Ensure accurate EMR documentation and participate in interdisciplinary rounds
Required Qualifications:
-
Active NY State RN license
-
Minimum 5 years of RN clinical experience
-
Strong background in care coordination, utilization review, and discharge planning

ASN/ADN from an accredited nursing program
-
BLS certification
-
PRI and Screen certification (within 3 months of start)

Primary source verification via NYSED
Preferred:
-
Leadership or supervisory experience
