What You'll Do

• Deliver skilled nursing visits including wound care, catheter care, and medication administration
• Monitor patient vitals and health status, flagging changes to the supervising RN or physician
• Educate patients and families on medications, diagnoses, and how to thrive between visits
• Implement individualized care plans under RN supervision and collaborate with the full care team
• Coordinate with RNs, therapists, and home health aides to keep care seamless and patient-focused
• Keep documentation accurate and timely because great care deserves a great paper trail

What You Bring

• Active LPN license in the State of Florida (Compact license accepted)
• 1+ year of home health or clinical nursing experience preferred
• Current CPR/BLS certification
• Valid Florida driver's license and reliable transportation
• Proof of TB test, required immunizations, and health clearance
• Strong communication skills and a patient-first mindset
