We are a non-profit long term care facility that offers independent living, assisted living, and skilled nursing care. Job Summary:

Assist the Social Services Manager and Social Service Coordinator in planning, organizing, and implementing the overall operations of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, established policies and procedures of The Sharon at SouthPark, and as directed by the Social Services Manager, and/or the Administrator, Manager or Director or Nursing to assure that the medically related physical, mental, emotional and social well-being of the resident is met/maintained on an individual basis.

Essential Functions:

  • Assist with and support the initial resident admission process, as directed, to facilitate a positive move in experience for the resident to healthcare or assisted living.
  • Engages with the healthcare and assisted living residents daily to help them adjust positively to living in the facility.
  • Responsible for the planning, development, and implementation of assigned Social Services documentation, such as admission / quarterly assessments, progress notes, care plans, behavior progress notes, BIMS, in the EHR (electronic health record) in accordance with current federal, state, and local standards, guidelines, and regulations and department policies of The Sharon.
  • Ensure that all documented social services progress notes in the EHR (electronic health record) are informative and descriptive of the services provided and of the resident's response to the service.

Education and Experience:

  • At a minimum a high school diploma is required.
  • A baccalaureate degree in social work or a related human services field is strongly preferred.
  • The candidate must have a minimum of 6 months experience in a human services setting working directly with patients or clients.
  • 1 -2 years' experience of the candidate working directly with the senior adult in a health care or assisted living setting strongly preferred.
  • Ability to work with and understand the needs of residents requiring minimal to total care.
  • Ability to communicate effectively; verbally and in writing.
  • Ability to listen and respond to the needs of the residents.
  • Strong customer service skills.
  • Ability to function effectively with a team environment.
  • Willingness to work in an environment that changes daily based on the needs of the residents.
  • Proficient computer skills in Microsoft Office (Word, Excel, PowerPoint, Outlook) plus the willingness to learn other computer programs as required.
  • Ability to work independently or with minimal supervision.
  • Must conduct all business with a professional manner and with a high level of confidentiality.
  • Incumbent is authorized to take reasonable action necessary to carry out assigned responsibilities provided such action is consistent with Sharon at SouthPark's policies and procedures and sound management practices.
  • Ability to exercise judgment and discretion in establishing and maintaining positive working relationships.
  • Ability to research information and analyze data to arrive at valid conclusions, recommendations, and plans of action.
  • Recognition that errors in judgment may jeopardize the health and safety of the residents.
  • Recognition that errors in judgment may result in legal action against The Sharon at SouthPark.

Physical Requirements:

Works in an office type setting, extensive walking throughout the facility. Prolonged periods of sitting documenting in medical record. Requires verbal and written communication in English. Must be able to handle a fast-paced environment, moving independently from one location to another.
