Job DescriptionThe Social Worker is responsible for supporting residents and their families throughout their stay. This role combines compassionate support, strong case management skills, a desire to perform advocacy services on behalf of the residents to ensure that concrete needs are met, as well as clinical social work services related to issues affecting them, knowledge of the NJ Department of Health and Senior Services governing regulations.

Responsibilities
- Provide compassionate counseling, guidance, and support to residents and families during adjustment to the nursing home environment.
- Advocate for resident rights and work to resolve concerns or conflicts promptly and professionally.
- Act as the facility Grievance Officer.
- Conduct psychosocial assessments and maintain thorough, accurate documentation in compliance with federal, state, and facility guidelines.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary care team.
- Coordinate with insurance companies and payers regarding coverage, authorizations, and required documentation.
- Participate in care plan meetings and interdisciplinary team discussions.
- Manage the discharge planning process for sub-acute rehab residents, ensuring appropriate services, equipment, and resources are arranged for a safe transition.
- Maintain current knowledge of community resources, support services, and regulatory requirements.
- Participate in QAPI initiatives and procedures.
- Conduct yearly staff education trainings in topics such as abuse, neglect or misappropriation, Dementia, etc.
Candidate Requirements
Essential
- Bachelor’s Degree in Social Work (BSW) and current certification or licensure by the New Jersey State Board of Social Work Examiners.
- Must be able to read, write, understand and communicate effectively in English and interact tactfully with elderly residents and their families.
Preferred
- Minimum 1 to 2 years of experience in a health care, skilled nursing, or long-term care setting preferred.
- Exceptional communication, organizational, and interpersonal skills.
- Proficient in PCC electronic health record system, E-fax, Microsoft Office programs, and the Google Suite.
Requirements
