Job Description

We are seeking a dedicated and knowledgeable pharmacist to join our team, focusing on providing exceptional home care pharmacy services. The pharmacist will assess patient suitability for home care, design and monitor pharmacy care plans, and ensure the effective and safe administration of medications. This role involves collaboration with healthcare professionals and direct patient interaction to ensure the highest quality of pharmaceutical care.

Responsibilities

  • Collect and organize all patient-specific information needed to determine the suitability of patients for home care.
  • Assess patients' suitability for home care in accordance with established policies.
  • Determine medical condition and prescribed medication therapy suitable for home care services with clearly defined outcome goals.
  • Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
  • Make suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters for a home care patient.
  • Collect and organize patient-specific information to prevent, detect, and resolve medication-related problems.
  • Identify appropriate monitoring parameters for each patient.
  • Modify pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information.
  • Provide education to staff, other professionals, home care patients, and/or their caregivers.
  • Use effective patient education techniques to provide counseling to home care patients and/or their caregivers.
  • Ensure continuity of pharmaceutical care to and from the home and other patient-care settings.
  • Instruct pharmacy technicians on how to prepare medications so they are appropriately concentrated, stable, and appropriately stored.
  • Dispense medications using appropriate techniques and following established policies and procedures.
  • Adhere to safety and quality assurance practices.
  • Prepare labels that conform to established policies and procedures.
  • Inspect the final medication before dispensing and ensure authorization to dispense.
  • Comply with state/federal laws and USP standards, including DEA controlled substance regulations.
  • Manage the use, maintenance, and troubleshooting of medication administration equipment.
  • Participate in the process for assessing, managing, and reporting medication-related incidents.
  • Provide timely responses to requests for drug information from healthcare providers and home care patients.
  • Collaborate with support staff and other stakeholders effectively.
  • Prioritize workload and organize workflow, ensuring accuracy of pharmacy support staff work.
  • Maintain confidentiality of patient and proprietary information.
  • Comply with accreditation, legal, regulatory, and safety requirements.

Essential Skills

  • Experience in home infusion pharmacy (not just retail or inpatient).
  • Experience with CPR+ (home infusion-specific EMR is a major plus).
  • Active MD and DC pharmacy licenses.
  • At least one year of experience and strong knowledge base in sterile IV admixture procedures.
  • Ability to problem solve and work in a high-stress environment.
  • Computer literacy.
  • Excellent problem-solving, organizational, and communication skills.
  • Ability to exercise independent judgment.
  • Knowledge of Performance Improvement Principles and Practices.
  • Competence in the use of pharmacokinetic principles and formulas.

Additional Skills & Qualifications

  • Ability to maintain required licensure.
  • Competence in assessing patient laboratory values.
  • Participation in educational seminars or other CEU opportunities to promote professional competency.

Work Environment

The pharmacist will work in a small closed-door pharmacy, ensuring privacy and focus on patient care. The role involves handling medication administration equipment and collaborating with a team to support the infusion process at home for patients.

Job Type & Location

This is a Contract to Hire position based out of Owings Mills, Maryland.

Pay and Benefits

The pay range for this position is $68.00 - $72.00/hr.
