Position Type: Mobile: Full-Time (40hrs)/Non-exempt

Job Summary: MNRAAA is seeking a full-time Person-Centered Counselor (PCC) to help Veterans who need assistance choosing their caregivers, services, and supports so they can safely live in their homes. A PCC assesses the functional status of the Veteran and identifies areas of need and preferences to meet those needs. They develop and help implement a care plan that meets the Veterans' identified needs and does not exceed their budget allocation. They perform ongoing monitoring of services and support to ensure the Veteran's needs are met. Requires a valid driver's license, proof of insurance and a reliable car.
Salary and Benefits:
- Salary range: $22.04-$28.94 per hour, depending on years or relevant experience.

Key Responsibilities:
- Assess Veterans to determine their preferences and needs in order to develop a person-centered care plan.
- Provide support to the Veteran in understanding, enrolling, and managing the Veterans Directed Care program and budget.
- Coordinate with Veteran Affairs Medical Centers and fiscal agent to ensure success of program.
Qualifications:
- Requires a bachelor's degree from an accredited college or university in a human services-related field, such as social work, nursing, public administration, or gerontology and one year of professional or internship experience working with older adults.
- Requires interpersonal skills to interact with individuals from varied backgrounds, viewpoints, and experience levels, including co-workers, board members, clients, etc.
- Requires effective phone-based and in-person interviewing, listening, and person-centered assessment techniques including handling calls.
