- Job Title: Home Health RN
- Location: Kentucky – Counties Covered: Daviess, McLean, Ohio, Webster, Henderson
- Shift: Monday–Friday, 8:00 AM – 5:00 PM
- On-Call: Every 3rd or 4th weekend
- Pay: $53.00-56.00 an hour paid weekly + mileage reimbursement
- Benefits: available to full-time employees after 90 days of employment
Position Overview:
We are seeking a skilled Registered Nurse (RN) with home health experience to provide comprehensive case management for high-risk members. The RN will perform assessments, coordinate care, and collaborate with healthcare providers to ensure individualized, cost-effective care plans that support optimal patient outcomes.
Primary Responsibilities:
- Perform comprehensive assessments of high-risk members and develop individualized care plans.
- Collaborate with primary care providers, social workers, and other healthcare professionals to implement interventions.
- Ensure compliance with treatment plans, including non-medical support such as housing and transportation.
- Coordinate care transitions and engage specialty resources as needed.
- Document clinical, functional, and fiscal outcomes thoroughly.
- Identify opportunities for health promotion and illness prevention.
- Advocate for patients while maintaining confidentiality.
- Meet productivity requirements: 30 points per week / 6 points per day.
Qualifications:
- Current, unrestricted RN license – Kentucky required
- Associates or Bachelor’s degree in Nursing or related field
- Minimum 1-year home health experience preferred
- Skilled in IV therapy, wound care, wound vac management, and other RN competencies
- Experience with Homecare Homebase (HCHB)
