Job Description
We are seeking a dedicated Licensed Medical Social Worker (LMSW) for a day shift assignment. In this role, you will support patients across Medical-Surgical, Transitional Care, and Emergency Department settings, providing essential psychosocial care and discharge planning. This is a weekday-only position with no call or weekend requirements, offering a consistent schedule and collaborative work environment.
Key Responsibilities
- Provide comprehensive psychosocial assessments and develop care plans for patients
- Support discharge planning and coordinate transitions of care
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Assist patients and families with emotional, social, and financial concerns
- Navigate community resources and connect patients with appropriate services
- Maintain accurate and timely documentation using Epic EMR
- Deliver support across multiple units including Med/Surg, Transitional Care, and Emergency Department
Qualifications
- Active Licensed Medical Social Worker (LMSW) license (state license required)
- Minimum 1 year of experience in a healthcare setting
- Experience working in Medical-Surgical and/or Emergency Department environments preferred
- Proficiency with Epic electronic medical records system required
- BLS certification required
- Strong communication, organizational, and interpersonal skills
- Ability to work effectively in a fast-paced, multidisciplinary environment

- 22-week contract position
- Shift is Day 5x8-Hour (08:00 AM - 04:30 PM), 40 hrs/week
- Location is Paynesville, MN 56362
- Pay Rate for Travelers: $2,300 - $2,350 per week
