Job Summary:
The Kidney Group Inc, Multi Physician Specialty Office, seeking a dedicated and experienced Nurse Practitioner (NP)to join our nephrology team. The NP will provide specialized care to patients with acute and chronic kidney diseases, including those undergoing dialysis, hospitalization and in an outpatient office setting. This role involves patient assessment, diagnosis, treatment planning, and collaboration with a multidisciplinary team to ensure high-quality renal care.
Key Responsibilities:
Patient Assessment:
o Conduct comprehensive evaluations focusing on renal health.
o Review patient histories, symptoms, and diagnostic results.
Diagnosis & Treatment:
o Diagnose kidney-related conditions such as CKD, ESRD, hypertension, and diabetes.
o Develop and implement individualized treatment plans.
o Prescribe medications
Care Coordination:
o Collaborate with nephrologists, dietitians, pharmacists, and social workers.
o Monitor treatment responses and adjust plans as needed.
Documentation:
o Maintain accurate and timely electronic medical records. Epic Background preferred.
Qualifications:
Certified Nurse Practitioner (NP) with Acute Care/ Internal Medicine experience preferred.
Active RN and NP Ohio and Pennsylvania license
BLS and ACLS certification
Minimum of 2 years of clinical experience in Acute / Critical Care, Internal Medicine or Nephrology preferred.

Strong clinical judgment and diagnostic skills
Proficiency in interpreting lab and imaging results
Excellent communication and patient education abilities
Ability to work independently and collaboratively
