As the Care Coordinator at Meals on Wheels Western Michigan, you will play a meaningful role in ensuring that older adults receive the services they need to remain healthy and independent. Reporting to the Home Delivered Meals Program Manager, you will be responsible for the in‑home and phone assessments of home‑delivered meal clients and will serve as a liaison between other organizations and internal MOWWM departments.
In this role you will:
- First and foremost, align with our mission and values as an organization.
- Conduct in-home and phone assessments of home-delivered meal clients determining eligibility, risk for food insecurity, and appropriate level of service
- Maintain accurate and complete client records, including entering information gathered from client assessments into electronic client records in real-time
- Assist clients with program navigation
- Address and problem-solve client concerns that may arise in a quick and efficient manner
- Acquire knowledge of current community resources and make appropriate referrals based on clients’ self-determination, including the reporting of suspected or confirmed abuse, neglect, or exploitation
- Act as a liaison between Meals on Wheels Western Michigan internal departments and other community resources on behalf of home-delivered meal clients
What you’ll Bring:
- Valid driver’s license and reliable transportation required, with the ability to travel throughout the designated service area daily
- Driving record acceptable to Meals on Wheels insurance company
- Strong time‑management skills with the ability to handle multiple priorities and appointments efficiently and reliably
- Strong knowledge of community resources
- Demonstrated commitment to providing excellent, compassionate customer service
- Current understanding of the physical, social, and psychological aspects of aging
- Access to a private home office space suitable for confidential work
- Exceptional communication skills, both written and verbal, with the ability to engage effectively with clients, team members, and community partners.
- Effective active‑listening skills
