Inpatient Pharmacist

Position Summary

We are seeking a dedicated and experienced Inpatient Pharmacist to join our pharmacy team. In this role, you will provide comprehensive pharmaceutical care to hospitalized patients by collaborating with multidisciplinary care teams, reviewing and verifying medication orders, counseling patients and caregivers, and ensuring the safe and effective use of medications. This position offers an opportunity to make a meaningful impact on patient outcomes in a dynamic inpatient setting.

Key Responsibilities

  • Review, verify, and dispense medication orders in compliance with hospital policies, state and federal regulations, and clinical protocols.
  • Perform clinical pharmacokinetic monitoring, therapeutic drug management, and medication therapy management (MTM) for assigned patient populations.
  • Collaborate with physicians, nurses, and other healthcare providers as an active member of the interdisciplinary care team.
  • Conduct medication reconciliation at admission, transfer, and discharge to ensure continuity of care and prevent medication errors.
  • Provide drug information, dosing recommendations, and clinical consultations to medical and nursing staff.
  • Identify, document, and report adverse drug reactions, medication errors, and near-misses in accordance with hospital reporting procedures.
  • Counsel patients and family members on prescribed medications, including proper use, storage, and potential side effects.
  • Oversee and mentor pharmacy technicians, students, and residents as applicable.
  • Participate in pharmacy and hospital committees, quality improvement initiatives, and formulary management activities.
  • Ensure compliance with Joint Commission, Minnesota Board of Pharmacy, and other regulatory standards.
  • Maintain accurate and complete patient medication records in the electronic health record (EHR) system.
  • Participate in on-call, evening, weekend, and holiday rotation as required.

Required Qualifications

  • Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited school of pharmacy.
  • Active, unrestricted pharmacist license in the State of Minnesota (or eligibility to obtain prior to start date).
  • Completion of an ASHP-accredited PGY1 Pharmacy Residency program, OR a minimum of 2 years of inpatient hospital pharmacy experience.
  • Strong clinical knowledge base with the ability to apply evidence-based medicine in a fast-paced environment.
  • Proficiency with electronic health record (EHR) systems and pharmacy information systems.
  • Excellent written and verbal communication skills and the ability to work collaboratively in a team environment.
  • Basic Life Support (BLS) certification (or willingness to obtain upon hire).

Preferred Qualifications

  • Completion of a PGY2 specialty pharmacy residency.
  • Board Certification (BCPS, BCCCP, BCOP, or other relevant specialty).
  • Experience with Epic, Cerner, or similar EHR and pharmacy management platforms.
  • Experience in a specialty area such as critical care, oncology, infectious disease, or cardiology.
  • Bilingual or multilingual candidates are encouraged to apply.

Work Environment & Schedule

This is an inpatient clinical role based in a hospital environment. The position requires the ability to stand and walk for extended periods and may involve lifting or carrying supplies. Pharmacists are expected to rotate through day, evening, and/or overnight shifts, including weekends and holidays, based on department needs.
