Job SummaryThe Licensed Practical Nurse (LPN) provides direct patient care in a variety of healthcare facilities, including rehabilitation centers, and long-term care settings. Under the supervision of registered nurses or physicians, the LPN is responsible for monitoring vital signs, administering medications and treatments, assisting with personal hygiene and mobility, documenting patient progress, and ensuring a clean, safe environment. The LPN plays a key role in supporting patient recovery and comfort through compassionate, efficient, and patient-centered care.
Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer medications and monitor patients for side effects or reactions
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with the supervisor

Qualifications

• Currently licensed in state of Massachusetts
• Previous experience as a Licensed Practical Nurse is preferred
• Excellent interpersonal and client care skills
• Valid driver’s license
• BLS/CPR certification required.
• Dementia Certification
