Summary:
The Home Care Coordinator manages client referrals and coordinates care services. This role requires
excellent organizational skills and the ability to communicate effectively with clients and staff.
Qualifications:
1. High School graduate or equivalent.
2. 1-year previous coordinator experience preferred.
3. Pass criminal background check.
4. English speaking, reading, and writing proficiency.
Job Responsibilities:
1. Receive, document, and track incoming referrals without delay.
2. Receives intake information and communicates with clinicians if clinical decision making is
necessary.
3. Manages all matters relating to referrals and communicates any change in client service
requirement, unsatisfactory staff performance or provision of services to the Agency Manager/
Director of Operations.
4. Advises supervisor on number of new cases daily, weekly, monthly to assist in program,
company development.
5. Review referral and billing data to ensure accuracy, assist billers as needed.
6. Communicates with the team and with field staff to assign new clients shifts promptly.
7. Handles any call outs by field staff to assign new staff to the open shift promptly.
8. Documents and communicates client status changes as indicated.
9. Documents all communication, complaints, incidents, and correspondence appropriately in the
clients medical record
10. Manages client inquires and concerns and directs them to appropriate agency staff. Ensure
timely follow-up on client and family concerns.
11. Attends agency meetings as directed.
12. Maintain a positive work environment by demonstrating and communicating in an effective
manner, and fostering positive relations with staff, clients, coworkers, and managers.
13. Follow all agency policies and procedures.
14. Adherence to state and federal regulations, including mandatory reporting laws and continuing
education requirements
15. Other duties as assigned by Agency Manager.
Physical Demands:
The employee should be able to travel and sit/ stand for extended periods. Employee must be able hear
and effectively communicate. Employee should be fit to work without restrictions.
