Job Description:\n\n POSITION SUMMARY  The Community Partnerships Department works directly with community-based organizations toward collective impact initiatives to fulfill Second Harvest Heartland’s Mission of “Ending Hunger Together”. Case managers will support Community Partnerships in the implementation of an economic mobility pilot by providing direct case management services through intensive assessment, motivational interviewing, goal setting, referral, and support services. This project is a macro-level intervention conducted through case management to support the economic mobility of program participants toward food security through a holistic case plan based on the social determinants of health.Services can be delivered in a wide variety of settings with a primary hybrid work environment and emphasis on telecommunication and in-person engagements in St. Cloud, MN within reason. Secondary to this role will be data collection and validation for service quality assurance and to reassess service delivery approaches as needed. Individual and group supervision will be offered to support licensing requirements as required by the Minnesota Board of Social Work. DUTIES AND RESPONSIBILITIESDevelop trusting relationships with assigned caseload to provide compassionate case management services that support individuals’ self-determined path toward economic mobility.Conduct assessments based on agency framework and utilizing motivational interviewing to support individual’s participatory treatment plan toward household stability.Maintain regular contact with caseload to monitor progress, conduct referrals, and advocate on behalf of individuals for services to support stability goals as needed. Document program participants’ progress throughout the duration of intervention period and ensure data validity to support participant outcomes and inform best practice recommendations.Collaborate with and support team to ensure participant needs and concerns are addressed in a timely manner in addition to greater team duties.Complete initial training and onboarding in line with program requirements with additional opportunities for professional development.Fulfill other responsibilities as assigned by Associate Economic Stability Projects Manager and other staff. ESSENTIAL QUALIFICATIONS:Access to a reliable form of transportation is required.  Comfort using modern technology, including computers, tablets/iPads and Microsoft Office Suite programs is preferred. Use of telephone/telephony is required.Time-management and organizational skills, including ability to work independently and self-motivate.A minimum current or provisional LGSW licensure within the state of Minnesota.At least one year of experience in providing case management services. DESIRED QUALIFICATIONSPrevious experience in supporting individuals in services for primary social determinants of health (housing, food security, healthcare, education, workforce training).Previous experience with non-profit organizations and general understanding of structure and operations.Applicants who are bilingual are strongly encouraged to apply. Some languages in demand: Spanish, Hmong, Somali, Karen and Russian.A language differential of $3/hour is available to bilingual candidates. TIME REQUIREMENTS AND SCHEUDLE:Total time requirement: 1 year; 40 scheduled hours per week. Orientation and Initial training: 1-3 full days in office in Brooklyn Park.Days of the week: Flexible. Generally, Monday through Friday with occasional evening and weekend hours.Time of the day: Flexible, scheduled in advance. Generally regular business hours 8:00am – 4:30pm; some evening and weekend opportunities.Work sites: Hybrid remote and Second Harvest Heartland office site in Brooklyn Park. If hired in greater Minnesota, there will be more travel/drive time. We are looking for Case workers (bilingual preferred but not required) in the St. Cloud areas.
