Job Description

  • Follow established standards of nursing practice and implement facility policies and procedures.
  • Maintain MDS schedule to meet both Medicare and OBRA requirements. Keep all disciplines informed of schedule and schedule changes.
  • Timely and accurate completion of MDS assessments. Assessments consist of residents, family, and staff interviews, physical assessments, and medical record review.
  • Timely submission of MDS assessments to QIES system.
  • Completion of Baseline Care Plans upon admission. Development and timely updates of resident comprehensive care plan.
  • Conduct bedside Admission Nursing Assessment, Braden Assessment, Fall Risk Assessment, Side Rail Assessment, Active TB Assessment, Admission Section GG Assessment, etc. on the day of admission.
  • Review admission chart for completion.
  • Audit skilled charts for completion of required documentation.
  • Gather information and facilitate Medicare meeting. Communicate discharge plans to interdisciplinary team.
  • Additional duties may apply.

Skills:

  • Proven experience as RN or similar role
  • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
  • Familiarity with health, safety and sanitation standards and procedures
  • Understanding of confidentiality obligations and nursing best practices
  • Excellent knowledge of medical and hospital terminology
  • Excellent communication and interpersonal skills
  • Compassionate and able to handle stress
  • Successful completion of RN license program is a must
