St. Vincent de Paul Rehabilitation and Nursing Center is seeking an experienced MDS Coordinator to join our nursing management team. As an MDS Coordinator, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments and care plans for our residents.

Responsibilities:

- Coordinate and complete MDS assessments and care plans for residents

- Ensure accuracy and completeness of MDS assessments and care plans

- Collaborate with interdisciplinary team to develop and implement resident care plans

- Monitor and evaluate resident progress and adjust care plans as needed

- Ensure compliance with state and federal regulations related to MDS assessments and care planning

Qualifications:

- Current RN license in the state of New Hampshire

- Minimum of 2 years of experience in MDS coordination in a skilled nursing facility

- Knowledge of MDS assessment process and care planning

- Strong organizational and communication skills

- Ability to work collaboratively with interdisciplinary team

- Knowledge of state and federal regulations related to MDS assessments and care planning

Requirements:

· Registered Nurse (RN) with a valid New Hampshire license

· RAC-CT certification preferred

· Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning

· Must have strong clinical and communication skills

· Extensive knowledge of the RAI process and reimbursement requirements for both federal and state

· Must be highly organized with the ability to multi-task throughout the day

· Experience with Microsoft PowerPoint, Word, Excel and Outlook preferred

· Experience in developing templates to utilize in facility for audit/educational use
