Job Description

Our Mission:

Enable individual persons who have serious mental illness, co-occurring substance use disorder and/or a dual diagnosis of SMI and IDD live a life that is meaningful to them in the community on terms they define.

Title: Social Worker (SW)

Supervisor: Program Director

Summary of Job Description:

Provide social work and case management services for Members in collaboration with families and other stakeholders to ensure coordination of care.

Specific Responsibilities:

  • Collaborate with admissions to support the admission and acclimation of new Members into the program setting.
  • Complete Incident Reports in accordance with processes outlined by the Managed Care Organization (MCO), State and Project Transition Policy and Process.
  • Collaborate with members of the multidisciplinary team to develop treatment plans for identified Members.
  • Complete monthly individual sessions with Members and document in DAP format within 24 hours of the session.
  • Provide documented case management services to Members including (but not limited to):
    1. Coordination of financial eligibility and benefits
    2. Coordination of family contact and support
    3. Conduct utilization and member care reviews with MCOs and other payers.
    4. Track member authorizations and input into Electronic Health Record (her) timely.
    5. Run authorization reports and other reports in the EHR as assigned.
    6. Oversee Level of Care changes and documented aftercare planning with Members.
    7. Rectify all billing issues in a timely fashion.
    8. Communicate with referral sources and other external supports (hospitals, rehabs etc.) to facilitate members successful return to the program site.
    9. Support Members with ongoing transition planning by way of the APPLE Plan.
    10. Complete discharge follow-up for all members according to PT/PCS process and procedures.
  • Facilitate/ Co-facilitate all groups as assigned and complete documentation within 24 hours of facilitating.
  • Support with transporting Members per the directive of the Program Director.
  • Support with observation of Member alcohol and drug screenings and document observations within 24 hours (if applicable).
  • Support with observing Member self-administration of medication as assigned.
  • Participate in phone coaching/ crisis management process, which includes (but is not limited to):
    1. Orienting Member and family to phone coaching process.
    2. Carry and respond to coaching phone calls on assigned days, including being available to go onsite for emergencies during hours when there is no or minimal staff coverage.
    3. Provide coverage for staff callouts.
    4. Following incident reporting and documentation process for all on-call emergencies.
  • Provide back-up support to the Program Director as needed for coverage.
  • Complete continuing educational requirements as aligned with regulatory compliance.
  • Attend individual supervision sessions and program meetings as scheduled by direct supervisor.
  • Other duties as assigned by the Program Director

Additional Performance Expectations:

  • Participate in multidisciplinary treatment team and will support and implement interventions and directives as directed by the Team.
  • Always demonstrate compassion and concern when supporting a Member through embracing Project Transition and PCS Mental Health’s Mission and Core Values.
  • Approach Member engagement from a non-judgmental stance understanding that a Member’s behavior is driven by experience, which may include trauma.
  • Treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions. Staff will show Members dignity and respect for their values and lifestyles.
  • Seek appropriate support, consultation with Clinician or Psychiatrist (if applicable), in conjunction with the Program Director or obtain supervision, when they are uncertain about how to respond or support a Member effectively.
  • Report back to the Treatment Team any observations of Member behavior that suggests Member may need additional treatment interventions and/or support.
  • Engage with all external parties/ individuals with professionalism and with a positive customer service approach, understanding that they are always representing the organization.

Qualifications:

  • Bachelor’s degree in social work or related field and 1 year experience in the Mental Health, Behavioral Health or Drug and Alcohol field or other bachelor’s degree with 2 years’ experience in the Mental Health, Behavioral Health or Drug and Alcohol field.
  • Two years of experience in case management, family support and collaboration of care.
  • Strong assessment and documentation skills
  • Excellent communication, interpersonal, organizational and time management skills.
  • Demonstrated ability to work as a part of a multi-disciplinary team.
  • High energy individual with strong work ethic and ability to multi-task.
  • Must be able to work in a self-directed manner.
  • Ability to maintain confidentiality.
