Status: Full Time

Shift: Friday & Saturday: 6:00 PM - 6:00 AM

Location: Monongahela, PA

About the Position:

Community Care, Inc. is seeking a compassionate and dedicated LPN/RN to provide one-on-one care for a wonderful 14-year-old male child in the Monongahela area. This is a rewarding opportunity to make a meaningful difference in the lives of our clients by providing personalized care and support.

Responsibilities Include:

• Assisting with all Activities of Daily Living (ADLs)
• Perform g-tube care/feedings
• Perform medication administration
• Monitor seizures
• Perform trach/vent care
• Maintaining clear communication with the care team and family

What do I need for this role with Community Care?

• Current Licensed Practical Nurse/Registered Nurse (LPN/RN) license in the state of Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health experience preferred).
• CPR certification (required).
• Reliable transportation and valid driver's license.
• Compassionate, dependable, and able to work independently in a home setting.
