Registered Nurse (RN) – Hospice Liaison

An Excellent Opportunity

Mountain Valley has an opening for a Registered Nurse Liaison who will work in the Winston-Salem service areas. As a hospice liaison you will have the responsibility for responding to hospice referrals, preparing with our care teams for services needed, and providing a physical assessment and admission to hospice as required.

Duties

  • Maintain a hospice presence in the area hospitals
  • Acts as a partner in care with physicians, case managers and other facility staff.
  • Educate patients and families about the hospice benefit and services.
  • Provides an initial physical assessment and admission to hospice as required.
  • Initiate the referral to admission process through gathering information, education, and form completion aspects of patient intake, admissions, and transition to home.
  • Perform hospice in place (hospital) admissions as needed.

Qualifications

  • Valid Registered Nurse License
  • BLS Certification
  • Valid Driver’s License

Schedule

  • Monday - Friday
  • 8am - 5pm
